Visa forms, passport forms, and the Service Request form you'll need to work with us.
The mandatory online form for every China visa applicant. Complete it at the official Chinese Ministry of Foreign Affairs website, but don't submit it until we've had a chance to pre-check it.
Complete your passport application form online at the U.S. State Department's website — this replaces the old handwritten forms. On the final "Print Summary" page, scroll to the bottom left and click Create Form to generate your printable application, then print it out.
Used by the U.S. Department of State when a passport applicant cannot establish their identity to the satisfaction of the person accepting the application. It's completed only by the identifying witness, who must have known the applicant for at least 2 years, be a U.S. citizen or permanent resident, and sign the form in the presence of a Passport Agent. The applicant must still present some form of their own identification.
Submit the Service Request Form online, print the confirmation page, and send it along with your application materials.
